Title: Case Manager
Program: Substance Abuse and Mental Health Treatment Program
Basic Function: The Case Manager provides face-to-face Case Management Services for persons with co-occurring disorders or solely diagnosed with a substance abuse or a mental health disorder. He/she takes a primary role in the maintenance and support of the customer's recovery.

Basic Function: The Case Manager provides face-to-face Case Management Services for persons with co-occurring disorders or solely diagnosed with a substance abuse or a mental health disorder. He/she takes a primary role in the maintenance and support of the customer's recovery.
Duties and Responsibilities:
- Assess needs and link consumers to appropriate public and private resources, provide adequate referrals
- Coordinate services with other treatment team members
- Maintain relationships with referral sources and other stakeholders
- Conduct case management meetings with the consumer in their home or at the office
- Work with clinical team to provide optimal client services, participate in weekly clinical meetings with team
- Network with the community and the city, attend relevant meetings and trainings on substance abuse treatment, mental health, homelessness, and health issues
- Conduct scheduled group substance abuse/mental health education sessions
Knowledge, Skills and Abilities
- General knowledge of homeless population health and substance abuse issues int he private and public sector
- Strong organizational skills and excellent time management
- Ability to work with individuals from a wide variety of racial, cultural, and socioeconomic backgrounds
- Ability to work and communicate effectively with other staff
- Ability to control and manage emergency situations and provide immediate assistance in crisis situations, support crisis intervention
- Ability to do an adequate screening of client and continuously determine needs, strengths, and resources of client
